January is our membership drive time!  Membership fee is $45.00 per person. We encourage both stroke survivor and caregiver to join! Funds collected are used to help cover the costs of our therapists, counsellor, hall rental and janitor service.

In order to ensure that we have your current information, please complete the form below. Please use one form for each member so each person is counted separately. The forms can be returned at a weekly meeting or mailed to 399 Dorset Rd, Qualicum Beach, BC V9K 1H5.  Don’t forget to include your payment (cash or cheque accepted).
